1. Acceptance of Terms

By downloading, installing, or using Form ("the App"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, do not use the App.

2. Description of Service

Form is a real-time exercise form coaching application that uses on-device pose detection to analyze body position, count repetitions, and provide coaching feedback during exercises. The App offers both free and premium features.

3. Subscriptions and Billing

3.1 Free Features

The following features are available at no cost:

  • 3 core exercises: Squat, Push-Up, and Deadlift Stance
  • Real-time skeleton overlay with color-coded form feedback
  • Automatic rep counting
  • Per-rep form scoring and coaching cues

3.2 Premium Subscription (Form Pro)

Premium features — including 6 additional exercises, progress tracking with calendar heatmap, trend charts, personal bests, and haptic feedback patterns — require an active subscription.

3.3 Pricing

  • Monthly: $2.99 USD per month
  • Yearly: $19.99 USD per year

Prices are in US dollars and may vary by region. Applicable taxes may apply.

3.4 Auto-Renewal

Subscriptions automatically renew unless cancelled at least 24 hours before the end of the current billing period. Your Apple ID account will be charged for renewal within 24 hours prior to the end of the current period at the same price.

3.5 Managing Subscriptions

You can manage and cancel your subscription at any time through your Apple ID Settings (Settings > Apple ID > Subscriptions). Cancellation takes effect at the end of the current billing period — you will retain premium access until then.

3.6 Free Trial

If offered, any unused portion of a free trial period will be forfeited when you purchase a subscription.

3.7 Refunds

All purchases are processed by Apple. Refund requests must be submitted to Apple directly through their support channels at reportaproblem.apple.com.

4. Camera and Device Permissions

Form requires camera access to provide its core exercise coaching functionality. You may deny camera access, but the real-time coaching features will be unavailable without it.

Camera data is processed on-device only using Apple's Vision framework. Frames are analyzed in real-time and immediately discarded. No video or image data is stored, cached, or transmitted.

5. Health and Safety Disclaimer

Form is not a substitute for professional fitness instruction or medical advice. Please consider the following:

  • Consult a healthcare provider before starting any exercise program
  • Form's coaching cues are automated suggestions based on joint angle analysis and may not account for your individual anatomy, mobility limitations, or pre-existing conditions
  • Pose detection has inherent accuracy limitations depending on lighting, clothing, and camera angle
  • You exercise at your own risk and are responsible for your own safety
  • Stop exercising immediately if you experience pain, dizziness, or discomfort

6. Content and Intellectual Property

All exercise definitions, angle rules, coaching cues, and other content provided in the App are the intellectual property of Jayden Luna. You may not reproduce, distribute, or create derivative works from the App's content without written permission.

7. Acceptable Use

You agree not to:

  • Reverse engineer, decompile, or disassemble the App
  • Use the App for any unlawful purpose
  • Attempt to gain unauthorized access to the App's systems
  • Redistribute the App's content or exercise data

8. Disclaimers

8.1 Pose Detection Accuracy

While we strive for accuracy, pose detection relies on Apple's Vision framework and is subject to environmental factors including lighting, camera angle, distance, and clothing. Form scores and rep counts are approximations and should be used as general guidance.

8.2 No Warranty

The App is provided "as is" and "as available" without warranty of any kind, express or implied. We do not guarantee uninterrupted or error-free operation of the App.

9. Limitation of Liability

To the maximum extent permitted by applicable law, Jayden Luna shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of or inability to use the App, including but not limited to physical injury, loss of data, or service interruptions.

10. Changes to Terms

We reserve the right to modify these Terms at any time. Material changes will be indicated by updating the "Last Updated" date at the top of this page. Your continued use of the App after any changes constitutes acceptance of the revised Terms.

11. Governing Law

These Terms are governed by and construed in accordance with the laws of the United States, without regard to conflict of law principles.
